12/2/2023 EnchantaVaughn Approved Upon arrival to the home setting, QMHPA opened up session with Ms. Broadnax and asked about any mental health emergencies at this time. Ms. Broadnax did not present any symptoms of SI or HI. QMHPA initiated session with Ms. Broadnax to assist her with dealing with healthy physical outlets. QMHPA began the session by asking Ms. Broadnax to discuss or identify the leading cause and/or triggers to her depressive symptoms causing her feel as though she has some health concerns. QMHPA educated Ms. Broadnax on how she should seek medical attention immediately as opposed to her just allowing the pain in her head to continue. QMHPA discussed with Ms. Broadnax as well as modeled appropriate ways to communicate with her family members and those that she comes in contact with. Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her by QMHPA. Ms. Broadnax voiced that she has to go to the doctor for a physical checkup and she fears the doctor because she never get good news about anything. Ms. Broadnax explained that she had been telling everybody that she has to go to the doctor but they never listen to her. Ms. Broadnax shared that she had gotten into an argument with one of her family members because she felt as though she was making it out to be nothing but cancer runs in their family. Ms. Broadnax stated “that is why a lot of times I rather just be in my house away from them. They are so damn sensitive and do not want you to say anything to them. I don’t have any one to be there for me. Ms. Broadnax communicated that she would try her best to limit confusion and conflict at this time.
